AFFIDAVIT

I, MAGSUCANG, ABNER T., Filipino, of legal age, married, and a resident of Bliss, Molave,
Zamboanga del Sur, Philippines, after being duly sworn to in accordance with law, hereby
depose and state:

That I am the BOD Vice Chairman of Rizal-Molave Multi-purpose Cooperative (RIMMPCO)


Rizal, Molave, Zamboanga del Sur from April 2011 up to the present;

That as such, I am receiving a monthly honorarium of One Thousand (Php1,000) Philippine


Currency;

That I have no other source of income aside from the above-mentioned honorarium;

As such, I am executing this Affidavit to attest to the truth of the foregoing and for whatever
legal purpose this affidavit may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of April 2012 at Molave,
Zamboanga del Sur, Philippines.
