ISSUE: CAR 61.06 (Skill test form must be submitted within 30 days of the skills test) Yes No

1. Applicant 18 years or older x


2. Valid Class 1 medical certificate issued in terms of Part 67 x
3. Hold a valid general certificate of proficiency in radiotelephony x
4. Completed and signed relevant application form
5. Show evidence of holding a valid Student Pilot Licence (If a student on an integrated course) or having held,
within the previous 60 months, any of the following -
(i) a South African PPL (Helicopter)
(ii) a pilot licence (aeroplane) issued by a Contracting State; or x
(iii) an SPL where the applicant has completed an integral training course approved by the Authority
6. Have successfully completed the training as prescribed in Document SA-CATS 61 at an approved Part 141
aviation training organisation
7.
Have passed the theoretical knowledge examination as prescribed in Document SA-CATS- 61
8. Have undergone the skills test referred to in regulation 61.06.4 within 36 months of passing the examinations
referred to in (6) above and within 30 days of the last period of dual instruction
9.
Outcome of skills test must be marked on skills test
10.
Skills test initialed on each page by the DFE and Applicant

11. Initial skills test must be undertaken in a helicopter of not less than 500 Kg MCM

12. Certified logbook copies including career summary and endorsement of skills test by Instructor/DE on the logbook
A minimum of 200 hours total flight time (May include 20 hours flight instruction time in a helicopter FSTD

14. A minimum of 150 hours total flight time if a student has successfully completed the relevant on an integrated
course (May include 30 hours of flight instruction time in a helicopter FSTD approved by the Director for the
purpose)

17. Minimum of 20 hours Cross Country time as PIC x


18. Appropriately valid rated DFE checked

19. Sign next to all amendments and no tippex allowed

20. The appropriate fee as prescribed in Part 187

21. Show proof of English Language Proficiency in terms of CAR 61.01.7 x
