Dear Mr. Bagapuro,

Please accept this letter as my formal notice of resignation from my current position as IT Field Engineer
with 5L Solutions Supply and Allied Services Corp. effective from 30 days from today’s date (December
19, 2021) due to personal and health reasons.

I will continue to perform my regular duties until the effective date of my resignation. If there are any
exit procedure that I need to be aware of, please let me know. If I have outstanding leave credits, please
advise me how much I have and whether I am permitted to take it.

I would like to thank you for having me as part of your team. Thank you so much for all of the great
opportunities for personal and professional growth that I have experienced and I will always be grateful
for lessons that I gained while working here.

I wish the company and all its employees nothing but continued success in the future. Thank you for
