AFFIDAVIT

I, Jonjo A. Manuel of legal age, Filipino citizen, and with the address at Unit 6 7/F Eastfield
Center Macapagal Avenue, Pasay city after having been sworn in accordance with law do hereby depose
and say;

1. That I have a pending application for Occupancy permit from the City Engineers Office, Pasay
City, Metro Manila for the construction of my Office Fit-out located at Unit 6 7th Floor
Eastfield Center, Macapagal Avenue, Pasay City
2. That I secured the service of Engineers in-charge, Engineer Charles Jaco Santos, Engineer
William J. Juan, Engineer Domingo L. Lagman and Engineer Jomar P. Nerona to facilitate the
processing of all documents and papers and supervise the above project, however the said
engineers are no longer interested to coordinate with me and continue the construction of my
residential building;
3. That I am executing the affidavit to inform the proper authorities particularly the Pasay City
Engineers Office that I already cancelled their service with me and hired the serves of
Engineer Adiator R. Soriano, Rogelio T. Cabanilla, Alfredo B. Tilan and Ace N. Rodriguez
4. That I am executing this affidavit to attest to truth of the foregoing statements and facts and for
whatever legal interest it may serve;
