AFFIDAVIT OF LOSS

I, Name, Filipino, of legal age, single, and a resident of Address,


Philippines, after being duly sworn in accordance with law, depose and state:

1. That I am the sole proprietor of Company a business duly organized


and existing under and by virtue of the laws of the Republic of the
Philippines;

2. That being registered with the Bureau of Internal Revenue, Company


has been issued a BIR Certificate of Registration; and subsequently,
the said business has printed 24 booklets of Sales Invoices numbered
from 0001-1200;

3. That sometime around the quarter of 2023, the original copy of the


said BIR Certificate of Registration and my Sales Invoices were
missing and could not be found;

4. Despite of diligent search and efforts, the same could no longer be


found such that I now believe that it is now lost beyond recovery;

5. As such, I am executing this Affidavit of Loss to attest to the


truth of the foregoing and for whatever legal purposes that this may
serve.
