WAIVER FORM

I, Gennelyn L. Israel , from BSECE 4-1 , hereby acknowledge


and accept full responsibility for my actions during practices involving the development of concepts,
choreography, and any other activities related to the PUP ECERA Dance Fest: ECEyaw Mo 2023 event. I
understand and agree that neither the Electronics Engineering Students' Society nor the PUP ECERA
Dance Fest 2023 committee shall be held liable for any consequences arising from my actions during the
event.

By signing this document, I willingly and voluntarily waive any claims against the PUP
Electronics Engineering Students' Society and the PUP ECERA Dance Battle 2023 committee in the
event of any undesirable incidents. I fully comprehend that I am solely responsible for my own conduct
and actions.

Moreover, I grant full authority to the PUP ECERA Dance Battle 2023 committee to exercise
their rights and take legal measures if circumstances arise that are beyond their control and require
intervention.
