issuance of my grandmother’s death certificate in support of our application for Tax
Amnesty before the Bureau of Internal Revenue (BIR) Pagadian City, Zamboanga Del
Sur;
3. I hereby undertake to be solely responsible for the contents of my Affidavit in case
of any resulting damage or prejudice and hereby further hold the government of
PSA free therefrom;

1. I am the grandson of ALILANG IMBING KANGAD the immediate and living nearest
kin;
2. I hereby authorize KENT A. CASTRO to transact, request, apply, fees, sign and other
acts relevant thereto with the Philippine Statistics Authority (PSA) for the issuance of
my grandmother’s death certificate in support of our application for Tax Amnesty
before the Bureau of Internal Revenue (BIR) Pagadian City, Zamboanga Del Sur;
3. However, the name printed in the document of Special Power of Attorney (SPA) was
“ROLAND” which was erroneously printed instead of “RONALD” which is the correct
spelling thereof;
4. I am executing this affidavit to correct the same and that I could not further correct
the spelling of the Special Power of Attorney since the signatories of the said
documents are already out of town and we will have to wait for them for a long time
to go back to our place at Lapuyan, Zamboanga Del Sur;
5. I hereby undertake to be solely responsible for the contents of my Affidavit in case
of any resulting damage or prejudice and hereby further hold the government of
PSA free therefrom;
