AFFIDAVIT OF GUARDIANSHIP

I, MYLA A. SAGONOY, of legal age, Filipino citizen and resident of Brgy.


Genagasan, Silvino Lobos, Northern Samar, after having been duly sworn to in
accordance with law do hereby depose and say:

1. That I am the legal guardian of LEOBERT A. SAGONOY who was


born on January 5, 2010, LYRA A. SAGONOY who was born on
October 24, 2011 and LEORENCE A. SAGONOY who was born on
February 18, 2016, who are the children of my children with my late
husband ANDY S. SAGONOY;

2. That from the death of my above named husband, until present I am


the one who is taking care of our said children;

3. That I am the legal guardian of our above named children and they
are under my care and custody;

4. That I am processing for the death claim of my above named


husband AFPMBAI in Quezon City;

5. That I am competent as their legal guardian and they are under my


care and custody;

6. That I am not an imbecile, insane, a vagrant or a vacuoles person or


a habitual drunkard or a habitual criminal, and have not abandoned,
neglected or refused to support said minor or caused him to commit
offences against the law.

IN TRUTH WHEREOF, we have hereunto set my hand this 28 th day of


March 2023 at Catarman, Northern Samar.
