1. About the project

The Government of Gujarat and Government of India has been implementing various
developmental schemes to improve the living standard of the people of the country. If
these schemes are implemented properly, all round development of the people will take
place which can lead to improve their leaving standard. But it is a reality that, due to lack of
knowledge about the various schemes, the benefit of the schemes is not reaching to the
real beneficiaries.

Keeping this in view the “SWAS” (SOCIAL WELFARE AWARENESS SCHEME) project was
started by The Young Citizen of India Charitable Trust under the social responsibility of
Suzuki Motor Gujarat Pvt. Ltd. in the nine project villages namely Sitapur, Hansalpur,
Jalisana, Mera, Naviyani, Finchadi, Shankhalpur and Becharaji-Bechar.

The purpose of the “SWAS” project is to create awareness about various government
schemes, provide adequate information and guidance to the concern beneficiaries of the
project area so that, they can avail the benefits of the respective schemes.

2. Objectives of the project: -

a. To enhance awareness about various health, nutrition, education and


employment related schemes.
b. To facilitate coordination and cooperation among community and concern
departments to avail the benefits of the various schemes.
c. To enhance networking among elected members, community and concern
officers to ensure the benefits of the various schemes to community.

3. Project Activities

The SWAS project was started from 15-February-2022 and has received in-principal approval till
31-March-2025. As a part of the Project following activities were organized during reporting
period.

3.1 Appointment of the project staff

An announcement for the recruitment of the two approved posts of Project Coordinator and
Community Mobilizer under SWAS Project was published in Sandesh a Gujarati New paper on
Sunday 02/01/2022. As a part of the process 133 candidates registered their candidature out of
which 15 candidates were called for interview and out of them two were selected and Suzuki
Motor Gujarat Pvt. Ltd. was informed about the selection, Mr. Arvind B. Patel as a project
coordinator and Mr. Jaswant Goswami as a community mobilize, but Mr Arvind b. patel who
joined the project as a project coordinator who resigned on June 14, 2022 and from June 15
2022 Mr. Pintu. D. Khant joined as a new project coordinator in the SWASS project.

3.2 Orientation of project staff

The selected staff was oriented about the project and their role in it. As a part of their
orientation, they were sent to the various government departments to collect the information
about the various schemes. New project coordinator Mr. Pintu D. Khant went into the field
with a community mobilizer and understood working pattern of the project.

3.3 Dialogue with government officers

Mr. Kalpesh Mehta, Senior Manager, CSR ,Suzuki Motor Gujarat Pvt ltd .,Founder of Young
Citizen Dr. Amar Vyas, Nitin makvana and Community Mobilizer Mr. Jaswant Gosai had
meeting with District Development Officer Mehsana, Taluka Development Officer and
Mamlatdar of Bechraji Taluka. They were briefed about the project and they shared their
suggestion for implementation of the project. An introductory letter about the project was also
provided to them. As an outcome of the dialogue, an officer offered a required support in the
project. (Annexure – 1 later of DDO, Mehsana, Annexure-2 Later of TDO, Bechraji)

3.4 Village Level Meeting

Initially project was launched in selected five villages. A round table meetings were organized
in these villages .During these meetings dialogued were held among officials of the Suzuki
Motors company, representatives of the Young citizen, Sarpanch/ elected members and local
leaders. During this process they were oriented about the project, activities and their role in it.
Later one local leader meeting in Sitapur village and faliya meeting in Hansalpur village.

3.5 Pre-Test for IEC


Pre-testing of the pamphlet to be prepared for Information Education and Communication
under Shwas Project, which was revised and then will be sent for approval.

3.6 Approval of Final Budget and Establishment of new project office


Suzuki Motor Gujarat Pvt. Ltd. sanctioned the budget which was kept by Young Citizen of India
Charitable Trust on 20th June, 2022. After the appoval of the budget new poject office (B-111
Seth prahlad bazar, opp. Umiya vadi, Shankhalpur road, Bechraji) was established on 1 st July,
2022 with required accessories like computer, printer, scanner and furniture.

3.7 Meeting with Suzuki officials and Sarpanch


Mrs. Sonal Mehta (President) and Dr. Amar Vyas (Founder) went into field work to check
the ongoing activities of the SWAS project and done meeting with sarpanch, local leaders
and village people, after this, meeting was done with Mr. Kalpesh Mehta (Senior Manager,
CSR) in which there was discussion of evaluation of field work, meet new recruited staff,
talked about new project office and about futue plans.
4 Future plan
I. Prepare a pamphlet and sticker.
II. Organize meetings and disseminate pamphlets.
III. Meeting government officials and local Leaders.
IV. Continue process of filling up of new forms/ applications for availing benefits of
the schemes.

To submit the forms/ applications of the villages to the concern government departments.

Project Activity :-

"Swas" project office

Project Office (B-111 Seth Prahlad Bazar, Opposite Umiya Wadi, Shankhalpur Road Becharaji) received
approval from June, 2022 for construction of new office from Suzuki Motors Gujarat Pvt Ltd. Started on
1st July, 2022. Computers in office Offices were set up with necessary items like printers, scanners and
furniture. Coordinator Khand Pintubhai, Mobilizer Gosai Jaswantbhai and Project Manager Makwana
Nitinbhai were present in this production.
Village meeting finchadi:-
A women's meeting was organized at finchadi village on 07-07-2022. Village leaders and village women
participated in this meeting. In this meeting women were given information about the welfare schemes
of the government. How are these schemes implemented, Which documents are required under this
plan Information was given about all of them. What supporting evidence is required in the plans at this
meeting? Who is this scheme applicable to All those details were discussed in detail above. In this
meeting, along with those schemes, forms of schemes like old age pension scheme and widow pension
scheme were also distributed. Coordinator Khant Pintubhai and Mobilizer Gosai Jaswantbhai, working in
village breath project, took all the responsibility to ensure that the benefits of many schemes run by the
government reach the people and every needy people get the benefit of this scheme.

On date 13/07/2022, Finchdi Gram Panchayat Sarpanchshree and Talatishree AD Chaudhary met and
started work in Finchdi village. A letter was given regarding that. And discussed with Talatishree about
the welfare schemes of Sarkarsree And told him about this project, and how our organization is working
in the village and asked him to cooperate during this operation
Village Level Workshop:-

(A) Several developmental schemes have been implemented for the people of the Government under
the Swas Project. A program was organized with the beneficiaries through the help of Suzuki Motors
Gujarat Pvt Ltd.Swas Project Antargartha Young Citizen of India Charitable Trust, with the aim that the
villagers get the information about the entire schemes inspired by the government and the people from
the outskirts of the village can benefit from it.

(B) In the program, out of total 27 beneficiaries out of total 27 beneficiaries of the villagers who have
benefited from the welfare schemes of the government under the breath project, MD Sir of Suzuki
Motors Gujarat Pvt. The benefit papers of the incentive scheme were given by the hands of
Sarpanchshree, Talatishree, village leaders, beneficiaries and villagers were present in the programme.

(C) Managing the program Dr. Amar Vyas said that during the time of monarchy in the country, there
was a system of Mahajans, the best help the needy people and do public interest works like schools,
hospitals, etc. Presently Gujarat state is a welfare state which has about 25 different departments which
have more than 300 welfare departments. Schemes are implemented in rural and urban areas Lack of
documentation to convey the benefits of the scheme to the man of the village or due to some
unexplained reasons some beneficiaries are not getting the benefit which is the implementation of the
project by young citizens under the breath project started to fill the missing link with the support and
guidance of the officials of Suzuki Motor Gujarat Pvt Ltd. has been done.

 Work during strike done by Talati’s:


The role of the Talatishree of the village is very important in the work going on during the
SWAS project. Almost all forms are stamped and sent by Talatishree with his signature. But
Talati Shri's strike lasted for 21 days and ended on August 23,2022, which affected all
operations at the gram panchayat level, including the processing of beneficiary forms of the
schemes, due to which no more cases could be filed this month in our field practice area.
 Project activity:
1. Village level meeting and awareness activity-
Under the SWAS project, according to the gant chart, faliya meeting, local leader
meeting and women meeting were conducted in which people were given information
about the welfare schemes of the government and its necessary documents. In which
meetings were mainly held at Shankhalpur, Finchadi, Becharaji, Hansalpur.

2. District level and block level co-ordination-


For the implementation of government schemes under SWAS project, district and
block level coordination is met with the government officials and the issues of the
project are regularly discussed by writing letters to them for the purpose of early
clearing of the forms under the project and early benefits of the schemes. The details
of which are given below:

3. Staff meeting-
The President and Founder have two meetings every month with the staff of SWAS
project. In that regard, a meeting was held on 16/08/2022 and 28/08/2022 in which
the issues raised in the previous meeting were discussed, the work done as per the
Gant chart and the future planning for completing the remaining work was discussed.

4. Success Story:
Rawal Jesangbhai Khodabhai, Village: Hasalpur, Taluk: Mandal, District:
Ahmedabad. Jesangbhai had filled the old age assistance form twice before but his
form was returned due to wrong document. Then they met a member of our
organization and found that their form was being rejected as their age was wrongly
indicated in their Aadhaar card and election card. After that, they got an affidavit
based on the school leaving certificate received by the guidance institute, amended
their Aadhaar card, renewed their Aadhaar card and bank passbook xerox and
submitted their income copy, and monthly assistance of Rs.1000/- has been started.

On 02/09/2022, a board meeting was held at Bajania Vas in Becharaji village.


About 22 women were present. They were given information related to health and given
information to take out Ayushman card, and also gave information about how much rupees
benefits.

 On 05/09/2022 went to mandal taluka to collect widow and old age


orders, its operator Hasmukh Bhai gave us 17 old age orders from
Sitapur which we got.

Staff meeting-

The President and Founder have two meetings every month with the staff of SWAS project. In that
regard, a meeting was held on 22/09/2022 in which the issues raised in the previous meeting were
discussed, the work done as per the Giant chart,Account file and the future planning for completing the
remaining work was discussed.
WORKSHOP PROGRAM IN MERA VILLAGE:-

On 27/09/2022 a workshop meeting was held in Mera village in which Ayushman Card and Shram Card
were distributed. Dr. Amar Vyas Sir discussed the welfare schemes of the government, as well as
destitute, old age, and widow pension and crisis relief Plans were discussed. Also, member was
appointed vice-chancellor in the village. A committee was formed in Mera village under the “SWAS

PROJECT”, in which the members of the village have been appointed .


As the Ayushman Card worked well across Gujarat and the beneficiaries got more benefits,
Gujarat received the Ayushman Excellence Award across the country.

After completing the Base line survey in September, the beneficiaries were left with field
work in the villages, they had the task of distributing forms, and also gave them information
about the schemes, and also gave information about the necessary documents to the
beneficiaries.

A meeting was held with Mamlatdar Saheb and Nyaya Mamlatdar Saheb of Mandal
Taluka in which the project work was discussed. There was also a discussion for the order of the

beneficiaries of Hasalpur and Sitapur villages .


SHANKHALPUR:-
On 31/10/2022 Bajania Vas mahila meeting in Shankhalpur village in which women and
men were present in the number of 20 to 25. They were informed about the scheme, and
informed about the Ayushman card and how many rupees it will benefit the hospital.

On 24/11/2022 went to Patdi Taluka Panchayat and Taluka Vikas Adhikari office to give forwarding
letter. Also went to give form for widow and old pension of Naviani and Mera villages.

A faliya meeting was held in Shankhalpur village, in which 22 women and men of faliya were
present. In the meeting, the beneficiaries were informed about Ayushman card and shram card.
Information was also given about the proof required for the card.

faliya meeting was held in Naviyani village, in which 17 women of faliya were present.
Beneficiaries were given information about Ayushman Card and Shram Card. And information was given
about the benefits of Ayushman Card.

Talked to Sarpanch Saheb, Talati Saheb and other members in Gram Panchayat in mera village to
collect the documents of the beneficiaries of my village as soon as possible.
03/12/2022 Mehsana Office Meeting:-

A project review meeting was held at Mehsana office on 03/12/2022, in which the ongoing work
of the project was discussed and further work was planned, and the beneficiaries who have
benefited from the breath project There was a discussion about taking the Panchayat letter from
the Sarpanch of the Panchayat.

10/12/2022 Finchadi Workshop:-

Finchdi village organized a workshop program in which Dr. Amar Vyas,


founder of the Young Citizen Institute and Sonalben Mehta, president of the
organization, former village sarpanch Saheb and village leaders and village
beneficiaries were present in which Ayushman cards and Shram cards were
distributed which 62 Ayushman Card and 16 Shram Card beneficiaries were given
the card by the guests present.
10/12/2022 Becharaji office meeting:-

A meeting was organized at Becharaji office on 10/12/2022 in which the following issues were

discussed.

- Filing, making list of beneficiaries

- Bring carpet, mic, speaker.

-Format and take signatures of the beneficiaries.

-Determine village by day.

Organizing workshops in Sitapur

- New decoration in the office.

- Bring maps of each village.

NAviyani Gram Panchayat Leader Meeting:-


A leader meeting was held in Naviani Gram Panchayat in which Sarpanch Saheb, Talati
Saheb and members of Gram Panchayat were present. In the meeting, the beneficiaries who are
left without benefits in Naviani village and who do not know about the scheme were asked to
convey information through Gram Panchayat members. Asked to get the help of the members of
the Gram Panchayat.

A board meeting was held at Thakor Vas in Mera village, in which about 30 beneficiaries
come.Faliya meeting gave information to the beneficiaries in the meeting about the benefits of
widow and old age pension.
