Fluid Calculation

A 20 kg male patient presented to the emergency department with hypovolemic shock. 600 ml of fluids were administered initially to stabilize the patient. The patient requires a total of 1400 ml of fluids to rehydrate fully. The fluids should be administered over 48 hours, with 176 ml given per hour for the first 6 hours, then 98 ml per hour for the next 18 hours, followed by 89 ml per hour.

Bolus ( Resuscitation )

20kg x 20ml= 400 ml given within 5-10 min

Blood pressure not yet stable !

we can give from 10-20 ml so will give 10ml

20kg x 10 = 200ml

blood pressure stable

total bolus receive is 600 ml


Maintenance

10 x 4 = 40

10 x 2 = 20

Total=60 ml per hour


De cit

20kg x 100=2000ml and now we reduce the amount of uid that given in resuscitation so :

600ml - 2000ml = 1400 ml is the amount of uid that required to rehydrate the pateint .now how we give the 1400 ml ?

we have two rules

-First rule half the total 1400 ml =700ml given rst 6 hours so 700ml / 6 hours =116ml additional to the maintenance amount which is
60 ml so total 176 ml given each hour for rst 6 hours only and now we still have 700 ml and we still have 18 hours

so 700 / 18 hours =38 additional to the maintenance amount 60ml = 98ml given after the rst 6 hours

-second rule we divide 1400 ml by 48 hours then we add the result to the maintenance dose so

1400/48=29 + 60 = 89 hourly
