IOSH Risk Assessment Example 1

This risk assessment form identifies several hazards at an airport ATC work area: 1) Wet and slippery floors that could cause serious injuries from slipping, 2) Unsecured electrical wires that could easily trip employees, and 3) Stress from overwork without breaks that could lead to fatigue. Existing controls and additional controls to further mitigate the risks are identified. Actions to monitor the controls and hazards are assigned to cleaning staff, engineers, managers and AC technicians.
