Verify the order for an ECG in the patient’s medical record.
Gather all equipment
Perform hand hygiene and put on PPE, if indicated
Identify the patient
Ensures the patient’s privacy
Explain the procedure with patient
Have the patient lie supine in the center of the bed with the arms at the sides.
Place the ECG machine close to the patient’s bed, and plug the power cord
into the wall outlet.
Prepare the skin for electrode placement. (1)
a- If an area is excessively hairy, clip the hair. Do not shave hair.
b- Clean excess oil or other substances.
C- Remove any metal object from the patient

Apply limb electrodes, and connect the limb lead wires to the electrodes.
• Right arm (Red)
• Right leg ( Black )
• Left arm ( Yellow )
• Left leg ( Green )
Apply the chest electrodes
V1: Fourth intercostal space at right sternal border
V2: Fourth intercostal space at left sternal border
V3: Exactly midway between V2 and V4
V4: Fifth intercostal space at the left midclavicular line
V5: Left anterior axillary line, same horizontal plane as V4 and V6
V6: Left midaxillary line, same horizontal plane as V4 and V5
Press the AUTO button.
Observe the tracing quality.
When the machine finishes recording the 12-lead ECG, remove the electrodes
and clean the patient’s skin
Return the patient to a comfortable position
Clean the ECG machine per facility policy
Perform hand hygiene and documentation.
