Risk Assessment Form-2

1) The risk assessment form identifies four main hazards during the task of replacing 33KV overhead wooden poles with concrete poles: working at heights, lifting equipment, electrocution, and manual handling. 2) For each hazard, the form identifies who may be harmed, how they could be harmed, existing risk controls, risk ratings, additional controls to reduce risk, and new residual risk ratings. 3) Actions to monitor risks are also identified, including inspection of equipment, following procedures, training workers, and designating those responsible for monitoring controls.
