This is to signify intent to seek accreditation of the

Office for the purpose of the selection of members of the Naga City Youth Development
Council (NCYDC) as stated in Section 24 of the Rules and Regulations Implementing
Republic Act no. 10742, otherwise known as the “Sangguniang Kabataan Reform Act of
2015”

Attached herewith are the following as required under the said ordinance for our
accreditations:

2. Certification from our Punong Barangay/Office of Student Affairs on the

4. Our list of activities conducted in the last year;
5. Our Plan and Budget for the this year;
6. Our Certificate of registration (if accredited);
7. Our List of Officers and Members; and
8. Our filled-up NCYDO Profiling Form.
