(Undertaking)

I, MICHAEL U. SOLIS, single, Filipino, and a resident of 732 Molave


Street, Juna Subd., Matina, Davao City Philippines, after having duly sworn to in
accordance with the law do hereby depose and say that:

1. I am General Manager of Davao Virgo Security & Protective


Services, Inc., with main office located at 732 Molave Street, Juna
Subd., Matina, Davao City.

2. I undertake that the year 2009;

i. I remitted payments for the benefits, due to our


employees, with the Social Security System (SSS),
Home Development Mutual Fund (HMDF) , Phil-Health,
and employees Compensation Commission (ECC).

ii. I Fulfilled the remittance requirements for the Bureau of


Internal Revenue (BIR), in behalf of our employees, as
required by law.

3. I am executing this affidavit to attest to the truthfulness and veracity


of the foregoing statements and to inform the proper authorities
concerned of the matter and for such other purpose/s this may
serve.
