In summary of Discharge Teaching / Progress Evaluation for the patient, there are 7 steps

of guidelines, first is M for medications which is to instruct patient on the medication to be taken
at home medications regularly as prescribed. Instruct client to use pillbox to monitor medication
intake. Likewise, advise the client to use an alarm if it needs to be taken with meals. Second is E
for environment or exercises, ask for the client’s support system and advise the patient to reside
in a calm and peaceful environment to avoid stress and anxiety. Next is T for treatment which is
to encourage client to sleep early before dialysis sessions. Advise the client to take care of the
arm access for hemodialysis through avoiding putting too much pressure and instruct client to
avoid getting blood and blood pressure in arm access. While H for health teachings, infections
are common among people receiving dialysis, and can be very serious. To prevent infection on
the arm access for hemodialysis, advise the client to wash hands often or use hand sanitizer and
check the access daily and notify staff if they notice any of signs of infection such as redness,
unusual drainage or swelling. Fifth letter is O for observation or outpatient follow up which is to
instruct client on schedule of check-ups and laboratory tests to monitor renal function and
progress. Moreover, advise client to attend on the scheduled date of hemodialysis. On the other
hand, D is for diet, Advise client to watch out for their intake such as in water, sodium, protein,
phosphorus and potassium. Use phosphorus food pyramids to balance their intake every day.
Also, boil food to reduce phosphorus of the food such as meat, legumes, and vegetables. Lastly,
S for spiritual which is to encourage the client to continue with religious practices such as prayer
and meditation to strengthen connectedness and spiritual-virtue of the client.
