entitled ____________, caused the annotation or registration of Notice of Lis Pendens on the
title of the subject matter land; 4. That the aforementioned civil case has been dismissed and has
already attained finality as evidenced by the Order issued by Regional Trial Court Branch _____
of _____ and Certificate of Finality hereby attached as Annexes “A” and “B” respectively.
WHEREFORE, premises considered, it is most respectfully prayed that the Notice of Lis
Pendens appearing on page 4 of TCT-______ covering the above-stated property be ordered
