KNOW ALL MEN BY THESE PRESENTS:

That I, Marley G. Bonanza, of legal age, single, Filipino citizen and resident of Phase 1 Block 6 Lot
11 The Garden Villas Tanza, Cavite, do hereby name, constitute and appoint Shaine S. Quiros, of legal
age and resident also of 511 Prudencio St. Sampaloc, Manila, as my true and legal representative to act,
for me and in my name, place and stead, to do and perform the following acts, to wit:

To claim my SSS Partial Maternity cheque amounting to Fifteen Thousand Pesos (Php 15,000.00) at
Kuya J’s Main Office- Paco, Manila.

HEREBY GIVING AND GRANTING unto my representative full power and authority to do and
perform any and every act and thing whatsoever requisite, necessary or proper to be done in and about
the premises as fully to all intents and purpose as I might or could do if personally present and acting in
person; and

HEREBY RATIFYING AND CONFIRMING all that my representative shall lawfully do and cause to
be done under by virtue of these presents.

IN WITNESS WHEREOF, I have hereunto set my hand at Manila City, Philippines, on this 12th day of
