Dobutamine: Uses and Contraindications

Dobutamine is a positive inotropic drug that works by stimulating beta1 receptors in the heart to increase myocardial contractility and cardiac output. It has relatively minor effects on heart rate and arrhythmogenesis. Dobutamine is indicated for short-term inotropic support in cardiac decompensation due to depressed contractility from organic heart disease or cardiac surgery. Contraindications include hypovolemia, acute MI, general anesthesia with certain agents, and pregnancy. Side effects include increased heart rate, blood pressure, arrhythmias, and gastrointestinal symptoms. Nurses monitor patients closely during infusion and adjust the dose based on cardiac output and side effects.
