AFFIDAVIT OF IDENTITY

I, CRESENCIANA LIBO-ON y EUCOGCO, Filipino, of legal age, and a


resident of Brgy. Estefania, Bacolod City, Philippines, after having sworn to in
accordance with law, hereby depose and say:

1. That I was born on March 5, 1956 to spouses Alfredo Eucogco and Letecia
Belen - Eucogco at Binalbagan, Negros Occidental, Philippines;

2. That I am currently applying for the issuance of my Philippine Passport for


foreign travel purposes;

3. That all the information contained in the documents I submitted and


represented before the Department of Foreign Affairs-Bacolod are true and
correct which are the faithful representation of my true identity;

4. That although it appears in my Marriage Contract record that my name has


been entered as “Cresencia”, the same is a result of a negligible error by the
person who prepared thereof during our marriage;

5. That I undertake, under pain of perjury, that I am indeed, Cresenciana Libo-on


y Eucogco as represented in all the documents I submitted and that “Cresencia”
in my marriage contract is no other but me whose true name is “Cresenciana”.

IN WITNESS WHEREOF, I have hereunto affixed my signature this August 1,


2012 at Bacolod City, Philippines.
