AFFIDAVIT

I, DENIE ROY RAFOLS y Guarino, of legal age, Filipino and a resident of


Gardenville Subd., Tangub, Bacolod City, Philippines, after having sworn to in
accordance with law, hereby depose and say:

1. That I am the owner-proprietor of EDR COMPUTER CLINIC, a computer


repair and sale store located at 43P Hernaez St., Bacolod City, Philippines;

2. That I started renting and occupying the business space in September 2011 but
did not start the business operation until May 2012 because I enrolled in a
Basic and Advance Computer Troubleshooting course at Learn and Earn
Computer Center;

3. That in truth and in fact, EDR COMPUTER CLINIC HAD NO BUSINESS


OPERATION from September 2011 to April 2012, reason that I failed to file
my percentage tax return as there was no income during the said period;

4. That I attest to the truthfulness of the foregoing circumstances.
