1. A fatal incident occurred involving Mr. Jiten Gope, a 28-year old contractor employee, who suffered a crush injury to his waist while refueling a water tanker vehicle. 2. Mr. Gope was standing between the water tanker and a diesel filling vehicle when the parked water tanker suddenly started moving forward, running him over. 3. A preliminary investigation found that while the water tanker's engine was on and parking brake applied, wheel chocks were not used, allowing the vehicle to roll forward and cause the fatal crush injury.

What Happened:
On 30th May 2020, Mr. Nirajan Tiriya, driver of water tanker came along with Mr. Jiten Gope, the Injured Person (IP)
for diesel filling of water tanker’s fuel tank. Both, the diesel filling vehicle and water tanker were placed parallel to each
other. All the four persons, drivers and helpers of water tanker and diesel filling vehicle were standing in between the
two vehicles. While the IP was putting the cap of fuel tank, the water tanker suddenly started moving in forward direction.
During this injured person, Mr. Jiten Gope came in between front and rear tyre of the water tanker and was run over by
the vehicle. He was immediately taken to Tata Steel Noamundi Hospital for necessary treatment and during treatment
Injured Person succumbed to his injury.

Preliminary Findings:
1. The Injured Person (IP) had valid Medical examination and Vocational Training.
2. He was wearing all mandatory Personal Protection Equipment.
3. IP was standing in between water tanker vehicle and diesel filling vehicle
4. Water tanker vehicle was parked with engine “ON” and parking brake applied.
5. Diesel filling vehicle was in static condition.
6. Water tanker vehicle was parked on mine road having mild gradient.
7. Wheel chock was not placed below the water tanker vehicle tyre.

Immediate actions initiated


1. Injured person was taken to Noamundi Hospital for treatment with proper escort.
2. Incident site was cordoned off and all mining activities were suspended.
3. Statutory authorities and Sr. leaders were intimated about the incident.
4. The incident was communicated to all concerned at the mines.
5. Investigation team is formed for detailed investigation.

Mr. Jiten Gope is survived by his father Mr. Madhu Gope (55 years), wife Smt. Pinky Gope (25 years) and son
Rahul (9 years).
