being duly sworn in accordance with law, depose and state:

1. That I am a regular subscriber / user of a Smart Prepaid SIM Card with

2. That I usually put said sim card inside my cellular phone for use;
3. That I lost the said phone where the said sim card was inserted on Feb 5, 2019;
4. That despite diligence search and efforts to locate said card and phone, I could not find
it;
5. That I am executing this Affidavit of Loss to attest to the truth of the foregoing and in
support of my request /application for the replacement of the said SIM Card.
