I, ROLANDO SALES AGGABAO,of legal age, widow Filipino citizen and residents of
Pengue Ruyu, Tuguegarao City, Cagayan after having been duly sworn to according to law
hereby depose and state:

1) That I am the same person executing this affidavit;

2) That I was born to the late Manuel Albano Mallillin and Circy Sales Aggabao;

3) That I was born a male on August 12, 1949;

4) That I underwent sexual realignment in the United States of America;

5) That the same is not yet recognized here in the Philippines;

6) That I am executing this affidavit in order to attest the truthfulness of all the
foregoing statements and that we have done so freely voluntarily.
