I, AAA, of legal age, Filipino citizen, married, and a resident of


____________, after being duly sworn to on oath, hereby depose and say:

1. That I served as the barangay captain of _____________, from 2013 to


2018;

2. That during my term of office as barangay captain, I was not employed in


the government;

3. That as such, I did not receive any form of salary or compensation during
my term of office, except honorarium for holding a position in an ex-
officio capacity;

4. That I am executing Affidavit to attest to the truthfulness of the foregoing


and for all legal intents and purposes.
