9. What are the common sources of Medication Error? Give atleast two (2) and expound on
how to avoid these errors.
Common causes of medication errors include incorrect diagnosis,
prescribing errors, dose miscalculations, poor drug distribution practices, drug
and drug device related problems, incorrect drug administration, failed
communication and lack of patient education. These errors can be avoided
through proper endorsement or communication between doctors and nurses,
also, nurses to co-nurses during endorsement. Communication is always the key.
If a nurse is unsure on administering a certain medication, one should always
ask, or validate to his or her co-nurses, or superior, in order to avoid errors.
Before administering a certain medication, one should also check twice or even
thrice, so that we will be certain on the drug that we will administer. We as
nurses, should always know the indications, contraindications, classification, and
common side effects of the drug to be administered.
