Comprehensive Physical Exam Report

The physical examination showed the patient was cooperative with normal vital signs. On examination, the head, eyes, ears, nose, throat and skin were normal. The lungs were clear and heart had normal sounds with no murmurs. The abdomen was soft with normal bowel sounds. Extremities had normal muscle strength and range of motion. Neurological exam was normal with intact cranial nerves and reflexes. The reproductive exam was also normal. The overall impression was the patient was in active labor with rupture of membranes.
