KNOW ALL MEN BY THESE PRESENTS:

That I, Name of Principal, of legal age, ______ citizen, married/single and a resident of
______________________________, , after being duly sworn in accordance with law, depose and state
that:

I do to hereby name, constitute and appoint Name of Attorney-in-Fact to be my true and lawful
attorney-in-fact, for me and in my name, place and stead, and for my own use and benefit, to do and
