Soak

 Local application of water or a medicated solution


 can remove dead tissue or soften encrusted
secretions
 reduces pain and swelling of inflamed or irritated
skin surfaces

or abraded areas of the skin
Kinds of Soaks

 Saline – uses 4ml(1tsp) NaCL to 500ml


water(1pt). It has cooling effect, cleans and
decreases skin irritation.
Kinds of Soaks

 Oatmeal or Aveeno – uses 720ml (3cups)


cooked oatmeal in a cheesecloth bag. Tie the
bag securely and twirl it in the tub until the
water is opalescent.
-It soothes the skin irritations, softens and
lubricates dry, scaly skin.
Kinds of Soaks

 Cornstarch
- uses 0.45kg (1lb) cornstarch
in sufficient cold water to
dissolve it; then add boiling
water until the mixture is
thick and add to the tub water
- soothes skin irritation
Kinds of Soaks

 Sodium Bicarbonate
- uses 4ml (1tsp) sodium
bicarbonate to 500ml (1pt)
water, or 120-360ml(4-12oz)
to 120L (30gal)
- has a cooling effect and
relieves skin irritation.
Kinds of Soaks

 Potassium Permanganate – tablet form which are


crushed, dissolve in a little water and added to the
bath. It cleans and disinfects and treats infected skin
areas.
Sitz Bath

 indicated for the following clients:


- has undergone rectal or vaginal surgery
- post partum patients
- with local or rectal irritation (hemorrhoids, fissures)
 cleanse and reduces inflammation of the perineal and
anal areas, aids in wound healing
 induce voiding (urinary retention)
 relieves pain, congestion (post proctoscopy/cystoscopy)

 water temperature depends on the client’s condition but


should be 43°C-45°C(109.4°F-113°F).
 cold sitz baths are more effective in relieving pain in the
postpartum period.

◦ Gloves (if giving perineal care, presence of body
fluids or open lesions)

◦ Linens: bath blanket, two bath towels, washcloth,
clean gown or pajamas as needed, additional bed linen
and towels, if required.
◦ Personal hygiene articles (e.g. deodorant, powder,
lotions)

◦ Water between 43C-46C(110F-115F) for adults and
38C-40C(100F-105F) for children

WIPING THE EYE FROM

Tepid Sponge Bath

Purpose – to cool body slowly


Equipment
 Bath basin
 Tepid water (37°C or 98.6°F)
 Bath thermometer
 Washcloths
 Waterproof pad
 Bath blanket
 Thermometer
Procedure

1. Assess the client’s body temperature and


pulse.
2. Explain to client the purpose of sponging
with tepid water is to cool body slowly.
3. Prepare necessary equipment and
supplies.
4. Close room, door or curtains.
5. Wash hands.
Procedure

6. Place waterproof pads under client and remove


gown.
7. Keep the bath blanket over the body parts not
being sponged. Close the windows and door to
prevent draft.
8. Check water temperature.
9. Immerse washcloth in water and apply wet cloths
under each axilla and groin. If using tub, have
client soak for 20-30 minutes.
Procedure

10. Gently sponge extremity for 5 minutes.


11. Dry extremity and reassess client’s pulse
and body temperature. Observe response
to therapy.
Procedure

12. Continue sponging other extremities,


back, buttocks for 3-5 minutes each.
Reassess temperature and pulse every 15
minutes.
13. Change water and reapply sponges to
axillae and groin as needed.
14. When body temperature falls to slightly
below normal, discontinue the
procedure.

15. Dry extremities and body parts thoroughly. Cover


client with light bath blanket or sheet.
16. Dispose equipment and change linen if soiled. Wash
hands.
17. Measure client’s body temperature and pulse.
18. Record time procedure was started and terminated,
vital signs changes, and client’s response.
