claims to ownership except as shown on the application for title. I certify that the information I provided is true and accurate. I
am making this certification in support of my request for Oregon title under ORS 803.045 and ORS 803.050, and as proof of
ownership under ORS 803.205. I understand it is a crime under ORS 803.070 and 803.075 to knowingly make any false
statement relating to the application or assignment of a title to a vehicle. This offense is a Class A Misdemeanor and is
punishable by a jail sentence of up to one year, a fine of up to $6,250, or both.

Instructions
Use this form when you are applying for title on an assembled light trailer or an
assembled heavy trailer that has never been titled or registered before AND was
not manufactured by a commercial firm.
Complete all sections of the form and sign it.
Required documents are:
• A release of interest from whomever you purchased the trailer, if you didn’t build it; or
• Receipts or bill(s) of sale for both the frame and axle if you built the trailer.
If you cannot provide one of the above requirements, you must give a written explanation
in the space provided on the front of this form.
It is important that you complete the form thoroughly. Your transaction may be returned if
you do not provide the information needed to determine ownership or vehicle type.
A vehicle identification number (VIN) inspection is required. You must take the trailer to a
DMV field office for the inspection. There is a $7 fee for the inspection and be prepared
to pay the title fee. To determine if VIN inspections are available at the DMV field office

Each transaction is handled on a review basis only. Completion of this form does not
