Safety checks YES NA

1) Equipment drained , Cleaned and vented


2) Nearby fire hydrant are working , Hose& nozzle available
3) Combustible material & oils removed from the vicinity
4) Water sprayed for cooling the area
5) Regulators, gauges, hoses, torch , Cylinders, lighters and other Equipment in good condition.
6) Flash back arresters are used
7) Welding machine , Cut off switch , RCCB, Cables , return cable are in good condition
8) Gas cylinders in upright position
9) Fire extiguishers / fire buckets provided at work place
10) Return earth connected to welding machine
11) Combustible materials are protected with fire resistant sheets / fire blanket
12) House keeping restored
13) Fire watch present
14) Risk assessed and safety talk ( TBT given )

I have ensured that requirement for the application of the permit (S) stated are fulfilled . I have under stood the hazards associated with
the work and i assure that all safety rules and procedures will be observed as per the checklist while carrying out the above said job.
