AFFIDAVIT
(Out of Town Registration of Birth)

I, CLARITA N. PORQUILLANO, of legal age, married, Filipino and with


residence and postal address at #80 Barangay Sala, City of Cabuyao, Laguna, after
having been duly sworn to in accordance with law hereby depose and state:

That I am the daughter of Spouses FELICIANO PORQUILLANO and


JUAQUINA NARBARTE;

That I was born on January 15, 1963 in Pozzorubio, Pangasinan;

That the said birth certificate has no record in the office of the Local Civil
Registrar of Pozzorubio, Pangasinan;
That I wanted to register for the proof of birth;
That I am applying for the late registration of birth at the City Civil Registrar of
City of Cabuyao, Laguna through the Out-of-Town scheme.
That I am executing this affidavit to attest to the truthfulness of the foregoing
statements and for all legal intents it may serve.
IN WITNESS WHEREOF, I have hereunto set my hand this 22 nd day of June,
2020 at City of Cabuyao, Laguna, Philippines.

AFFIDAVIT OF CORRECTION/DISCREPANCY

I, PERLA DIAMANTE-BARRIENUEVO, of legal age, married, Filipino and with


residence and postal address at Block 34, Lot 30, Birmingham Village, Barangay Pulo,
City of Cabuyao, Laguna, after having been duly sworn to in accordance with law
hereby depose and state:

That I was born on September 2, 1968 in Pili, Camarines Sur as appearing in


public documents records;

That however, when I secured a copy of Certificate of Baptism issued by the


Saint Raphael the Archangel, I found therein that my name is erroneously registered
and indicated therein as “MA. PERLA DIAMANTE” instead of “PERLA DIAMANTE”
which is my true and correct name;

That the abovementioned mistake/error is plainly inadvertent and clerical;

That the person named “MA. PERLA DIAMANTE” appearing in the Certificate
of Baptism and the name “PERLA DIAMANTE” refer to one and the same person;

That this affidavit executed to affirm the truthfulness of the foregoing statement
and for any purposes it may legally serve.

IN WITNESS WHEREOF, I have hereunto set my hand this-10 th day of March,


2020 at City of Cabuyao, Laguna, Philippines.
