Greetings of Peace!

I, Gilbert Soriano Palaad, with reference driver’s license number E04-00-054261, authorize my
wife, Belen Baron Palaad with TIN number 491-983-308-000, to request, receive, claim and
transport the relevant legal documents that are necessary for my request – my mother’s death
certificate. For your information, please see the details of my mother below:

Mother’s Name: Evelyn Aragona Soriano Palaad


Born at: Quipayo, Calabanga, Camarines Sur
Born on: April 10, 1944
Died on: January 25, 1988
Died at: Cabusao, Camarines Sur

I hereby grant authorization to my wife - Mrs. Belen Baron Palaad to act and fulfill the
necessary obligations, on my behalf in the death certificate of my mother and its legal process.

To support my claim, I have two of the identity cards, respectively:

1. Driver’s License; and


2. Tax Identification Card.

I comprehend that, Mrs. Belen Baron Palaad will not be in any way, responsible for any
hindrance caused during receiving and processing.
