Information

The undersigned Assistant City Prosecutor of the City of Tacloban


accuses Lito D. Mapigilan for the crime Frustrated Homicide in violation of
Article 249 of the Revised Penal Code, committed as follows:

That on or about 3:00 o’ clock in the afternoon of May


24, 2018, in the City of Tacloban and within the
jurisdiction of this Honorable Court, the said accused,
allegedly with intent to kill, repeatedly stabbed one VIC
C. TIM on the stomach with a knife, thus performing all
the acts of execution which should have produced the
crime of homicide, as a consequence but which
nevertheless did not produce it, by reason of some causes
independent of the will of the accused; that is the timely
and able medical assistance rendered to said VIC C. TIM
which prevented his death.

The undersigned accuses KAYDEN MENDOZA, of the crime of


theft, committed as follows:

That on or about the month of February 2009, in the City of


Quezon City, Republic of the Philippines and within the
jurisdiction of this Honorable Court, the above mentioned accused
KAYDEN MENDOXA being an employee of TYESO INC. in
charge of operation of the machines for the production of water
tumblers with intent to gain, with grave abuse of confidence, did
then and there willyfully, unlawfully and feloniously take, steal
and carry away one thousand (1000) pieces of water tumblers
produced by the machines valued at Three Hundred Fifty
Thousand Pesos (Php 350,000.00), without the knowledge and
consent of TYESO INC. to its damage and prejudice.

I hereby certify that the accused was lawfully arrested without a


warrant and that, upon being informed of his rights, refused to waive the
provisions of Article 125 of the Revised Penal Code, and for this reason, an

presented before me without countervailing evidence submitted by the
accused, despite opportunity to do so, there is a reasonable ground to believe
that the accused has committed the crime of theft and should, thus, be held
