(FOR STUDENTS)

KNOW ALL MEN BY THESE PRESENTS:

That I, ______________________ a student of Dr. Carlos S. Lanting College for SY 2021-2022,


with address at ______________________________________________, hereby acknowledge
that I will be a recipient of the GSuite for Education , Cisco Webex and all other learning
platforms including social media that will be utilized by the faculty members of Dr. Carlos S.
Lanting College in the delivery of online and offline education.

That I hereby undertake to hold with utmost confidentiality everything confined in the GSuite
for Education , Cisco Webex and all other learning platforms including social media that will be
utilized in the delivery of my education .Therefore, I hereby undertake not to release or in any
way reproduce or share any part of the learning platforms, or to lend emails/username to any
person or to dispose of it in any manner including recording and taking videos of faculty
members while teaching without their permission.

I hereby acknowledge Dr. Carlos S. Lanting College right’s over the proper use of this learning
platforms. It is hereby understood that in case of breach or unauthorized release of confidential
information, unauthorized release , reproduction and sharing of the contents of the Learning
platforms in violation of this undertaking, I shall be held liable for whatever result, and must
face consequences that are based on the data privacy act and applicable institutional policy of
the Institution, in addition to payment of actual and compensatory damages as the institution
may suffer. In case my account gets hacked or cannot be opened, I hereby undertake to submit
a report to the Technical Management Services (TMS) to address the incident and I will be
responsible for my actions and be disciplined accordingly, if warranted by the circumstances.

That I hereby bind myself to be attentive in using the GSuite for Education and Cisco Webex in
our online class. It is hereby understood that I am qualified to use GSuite for Education and
Cisco Webex as Learning Management System (LMS) during our online class, and that I have
already received my account/username of GSuite for Education and Cisco Webex.

That I hereby give my consent to Dr. Carlos S. Lanting College to know my personal information
and use my picture in the course of using these online learning platforms. Finally, I hereby hold
the Institution free and harmless from any liability whatsoever arising from the Institution’s use
of my personal information, data and picture in connection with the use of GSuite for
Education, Cisco Webex and other Learning Platforms as Learning Management System (LMS).
