TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that FELIXBERTO S. BAS, JR. of legal age, married and resident
of Purok Tangkong, Barangay Hagnaya, San Remigio, Cebu has started attending the
Community-Based Rehabilitation and Treatment Program (CBRTP) in this Municipality
from September 12, 2020 until March 6, 2021.

This certifies further that Felixberto S. Bas, Jr. is among the other drug surrenderers
and plea bargainers who are enrolled to Katatagan Kontra Droga at Kriminalidad
(KKDK) intensive out-patient rehabilitation program, a training-based intervention
program of the Municipality of San Remigio with twelve (12) basic modules and
three (3) family modules. He has already completed the program and has shown
diligence and active participation throughout the entire 6-month training sessions.

As an additional condition in his availment of the plea bargaining program, he will


undergo Community Support, Aftercare and Reintegration Program (CSAR) in his new
residence in Maasin City, Southern Leyte.

This certification is hereby issued to form part of the required documentation for his
transfer of residence from Barangay Hagnaya, San Remigio, Cebu to Barangay
Cansirong, Maasin City, Southern Leyte and for whatever purpose this may serve
best.

Issued this 17th day of March, 2021 at Barangay Hagnaya, San Remigio, Cebu.
