Person-centred practice client 1
During my practice I have worked with different clients, however, I remember Tricia, an elder woman of 80 years
old. The first time that I met her I knew that she was a lovely grant mother, therefore, I realized that she wanted
the best attention from her support care worker. Tricia was looking to be helped every day with tasks like helping
her to go to the toilet, read a book for her and remind her the time to take her medicines. Therefore, the service
response was always to visit her every day from 8 am to 5 pm. During the visits, sometimes Tricia required new
changes within the services, for example, she asked me several times to take her outside to the mall, however, in
my position I was limited to do it. Thus, to deal with these changes I followed the codes of practices and I
contacted my supervisor to explain him about these changes requirements . Finally, it can be said that my client
never was unsatisfied with the service provided, she always understood that I was doing my best and that was
working within the legislative requirements. Furthermore, the client was happy with the monitoring systems (Ask
her every day about the service quality) that indeed helped to meet her needs.

The second client that I remember was Robert, a retired military who lived with his wife and two cute dogs. Robert
began to have problems to hear properly since some 5 years ago. Therefore, working with Robert was a real
challenge to me. His main needs were to help him to walk to the park near his house. While we were walking we
let the dogs run around which was a great time for Robert who enjoyed it. Furthermore, Robert asked within his
service to be helped by preparing him his food. As he had diabetes, I had to cook all his food as the doctor
recommended it. Finally, with this client I never had problems within the service offered, Robert was satisfied with
my service and he was happy to see how I keep asking him if the service met his needs and what else I can do to
improve it.

elder. Therefore, my service was every day to help him with tasks like leading him to take his showers, feeding him,
reading a book and share time with him. Once during my visit time, I saw that Alonso Mario’s older son was crying,
therefore, I asked him what happened, and he answered that he was worry because his dad did not talk with him
during the last 2 months and he believed that Mario had begun to forget him. Thus, I talked with Alonso to explain
out that his dad was sad because he missed his wife. Finally, Alonso understood it and the problem was overcome.

Working with Elena, a sweet elder who lived with her husband I was witness to an unpredictable problem when
one of her sons came to ask her to give money to buy drugs. At first glance, I continued working normally,
however, when Elena refused to give him the money he started to be loud and yelling her telling her that she must
provide that money. At that point, I ask him please to leave the house before I called the police. To be honest I was
scared, however, I call my manager to let him know about this issue. Elena’s husband was in shock; thus, I helped
him to calm down until the police came to take the control of this situation.

Once of my visits to Joshua a retired teacher who lived with his wife in a farm. During my visit I should to helped
him out Joshua cooking his food, reading books to him and feed him. Once, Margarete his wife was trying to move
a heavy bed form one room to other. Therefore, I did not doubt to help her and ask her please don’t do that effort.
Once done that task, she wanted to move the fridge too, by which a offered my help again. This has been the most
remarkable help in community setting.

Client 1 – socialisation, leisure and health
There was a client, Mario a 69 years old person with early problems of dementia. Therefore, the approach that I used
to take care of him was to be patient all times to give him the best service. Furthermore, as a support care worker I
was willing to spend extra time of my work to address all his needs.
I communicated with him orally, repeating always words and asking him twice to make sure that hi understood the
information.
I help Mario to take a shower, I cooked his food, I help him making his bed every day, reading books, setting the radio
for him and helping him to take his medicines.
The communication strategies were effective, Mario responded positively to the service.
Yes, Mario’s sons were totally happy with the activities developed every day within the take care service.
Perhaps, it would be good to provide a 24 hours attention to Mario, to make sure that he is helped all the time.

Client 2 – socialisation, leisure and health
Carla, a older woman with total dementia was the most challenging case for me. The communication with her were
verbally and by the use of signs. She hardly answered questions when I was doing the service.
What strategies did you use to assist in meeting a range of suitable needs?
I use many, for example, I always show her videos while I feed her, this strategy was effective. Furthermore, I read
books to keep her thinking awake during the day. Furthermore, as she had to be assisted in all activities, I show her
love by showing a big smile in every task that I did for her.
These strategies were highly effective. Carla sons were happy to see how her mum even with dementia was well fed
and clean. Moreover, it was notorious that Carla was more comfortable because sometimes she smile and dream
properly.
Perhaps, Carla would need two support care workers to address all her needs easier.
