I am Angela A. Bendecio authorizing my daughter, Maria Elena B. Rabor for


claiming my Birth Certificate in my behalf. She will present all the requirements
needed including my ID. I would like to ask for your cooperation since I cannot go
right now to your good office because I am having a hard time to walk long
distance.

Mrs. Rabor will bring my 1 valid ID for your future reference.
