SAMPLE GOALS AND OBJECTIVES

SMART TREATMENT PLANNING

Diagnosis:
Depressive Disorder (and Bipolar depressed)

Goal:
Resolution of depressive symptoms

Objectives:
 Patient will contract for safety with staff at least once per shift
 Patient will identify two coping skills related to (specific stressor)
 Patient will report at least six hours of restful sleep each night
 Patient will eat at least two out of three meals a day to gain weight
 Patient will attend at least two activities or groups a day
 Patient will reality test (specific delusion) with staff for 10 minutes a shift

Diagnosis:
Bipolar (manic)

Goal:
Resolution of manic/hypomanic symptoms

Objectives:
 Patient will report any perceived conflict to staff
 Patient will report at least six hours of restful sleep per night
 Patient will remain in at least two groups per day for the entire length of the group
 Patient will eat at least two out of three meals a day
 Patient will reality test (specific belief) for at least 10 minutes a day with staff
 Patient will participate in at least three groups/activities a day and comply with
group rules
 Patient will dress in an appropriate manner on a daily basis
Diagnosis:
Schizophrenia and Psychotic Disorder NOS

Goal: Resolution of psychotic symptoms

Objectives:
 Patient will contract for safety with staff at least once per shift
 Patient will report any perceived conflict to staff
 Patient will identify two medications and state why he is taking them
 Patient will participate in at least one complete group or activity per day
 Patient will reality test (specific belief) with staff for 10 minutes at least once per
shift

Diagnosis:
Schizoaffective Disorder

Goal:
Resolution of (specific symptoms)

Objectives:
 Patient will contract for safety at least once per shift with staff
 Patient will report at least six hours of restful sleep a night
 Patient will participate in at least two complete groups or activities a day
 Patient will reality test (specific delusion) for at least 10 minutes a day with staff
 Patient will eat at least two out of three meals a day
 Patient will identify two of her medications and state why she needs to take them

Diagnosis:
Anxiety Disorder

Goal:
Alleviation of anxiety symptoms and improvement in ability to function independently

Objectives:
 Patient will identify at least three new coping skills that she can utilize
 Patient will report at least six hours of sleep per night
 Patient will participate in at least two complete groups or activities per day
 Patient will identify feared situations and discuss at least one such situation in
daily 1:1 with staff
Diagnosis:
Obsessive-Compulsive Disorder

Goal:
Reduction in the amount of time spent focused on obsessive thoughts and performing
compulsive behaviors

Objectives:
 Patient will identify the relationship between obsessions and compulsions
 Patient will perform at least one new activity previously prevented by her OCD
 Patient will participate in daily relaxation group
 Patient will identify at least three behaviors with response prevention
 Patient will attend at least two complete groups or activities per day
