AFFIDAVIT OF DISCREPANCY IN NAME
I, EDENA LACDAO ZOSA, of legal age, widow and residing at B40 l14 Pegasus St.
Palmera Springs II, Barangay 173 Caloocan City, under oath, state:
1. THAT I EDENA LACDAO ZOSA wife of the late NAPOLEON M. ZOSA JR
child of the late MARIETTA Q. MACALUA and NAPOLEON B. ZOSA that
the name MARIETA Q. MACALUA that stated on the Death Certificate and
Cenomar and to NAPOLEON B. ZOSA SR stated on the Marriage Contract is
incorrect;
2. That the name MARIETTA M. ZOSA was the correct name instead of
MARIETA M. ZOSA, of my late mother-in-law stated on the Marriage Contract
and Birth Certificate while NAPOLEON B. ZOSA is the correct name instead of
NAPOLEON B. ZOSA SR of my late father-in-law stated Birth Certificate,
Death Certificate and Cenomar
3. THAT MARIETTA Q. MACALUA instead of MARIETA Q. MACALUA and
NAPOLEON B. ZOSA. instead of NAPOLEON B. ZOSA SR. is refers to one
person and the same person only.
That the discrepancy committed on the Names was due to mis
apprehension/typographical error. Said discrepancy was not intentionally done to deceive
or defraud any person, entity or organization and entered in good faith for no unlawful
reason or whatsoever.
. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and to
use the same for whatever legal purpose it may serve.

I, VIRGELIA CATUBIG TAGAPOLOT PAGPAGUITAN, of legal age, married,
Filipino with residence and postal address at 77 Faustino street, SMBAPI Compound,
Punturin, Valenzuela city, after having been duly sworn to in accordance with law, do
hereby depose and state, THAT:
1. I was born on June 26, 1964 at Hinundayan, Southern Leyte and one of the
children of the late Vicente Tagapolot and the late Estelita Catubig who died on
August 11, 2000 and January 7, 1998, respectively;
2. I am also one of the siblings of the late Vicente Tagapolot Jr. who died on June
12, 2019 at Valenzuela City;
3. The name of my father was inadvertently recorded as VICENTE SR SALABIT
TAGAPOLOT in the certification/negative copy of my late brother’s VICENTE
CATUBIG TAGAPOLOT JR. certificate of Live Birth
4. The recording was due to my honest mistake and not to commit any fraud to any
person or entity;
5. That on June 4, 1990, I got married to Edmond L. Pagpaguitan at Hinundayan,
Sothern Leyte;
6. In our marriage Contract my name was inadvertently recorded as VERGELIA C
TAGAPOLOT instead of VIRGELIA C. TAGAPOLOT my true and correct
name;
7. The recording was due to the honest mistake of the person recorded therein and
not to commit any fraud to any person entity
8. That the names VICENTE SALABIT TAGAPOLOT appearing in his
Certificate of Death, VICENTE SR SALABIT TAGAPOLOT JR appearing in
the certification/negative copy of my late brother’s certificate of Live Birth are
only one and the same person that is VICENTE SALABIT TAGAPOLOT;
9. That the names VERGELIA C. TAGAPOLOT appearing in my marriage
Contract and VIRGELIA CATUBIG TAGAPOLOT appearing in my
Certification of Live Birth are only one and the same person that is me;

1. That I JASMINE D. BARILE daughter of late JUANITO DILLEMA BARILE
that in my birth certificate issued by PSA my father’s middle name appears as
DELLIMA Instead of DILLEMA;
2. That my true and correct fathers middle is DILLEMA as could be attested by
her/his LOCAL CIVIL REGISTRAR
3. That JUANITO DILLEMA BARILE and JUANITO DELLIMA BARILE and
JUANITO DILEMMA BARILE refer to one and the same person only
4. That the discrepancy was due to excusable negligence on my part by the entry of
erroneous data on documents that I submitted to the above agency;
5. That it is my wish that the said discrepancy be corrected by the concerned
authorities subject to its rules and regulations so that the correct data will appear in
all papers, documents or records;
