EXPLANATION

I, CARLO ARBOLEDA, of legal age, single and a resident of Brgy.


Hanglid, Pres. Roxas, Capiz after being duly sworn in accordance with law hereby
depose and state that:

1. I received the Order of this Honorable Court dated November 8, 2019


only last November 22, 2019;

2. I am undergoing an Outpatient Treatment and Rehabilitaion in DOH-


TRC;

3. I humbly admit that I failed to religious comply with the schedule


provided by DOH- TRC for my Outpatient Treatment and Rehabilitation
in Manduriao, Iloilo City;

4. However, my absences are due to the fact that I am not gainfully


employed and I cannot sustain the expenses in going back and forth to
the rehabilitation center in Manduriao, Iloilo City. Considering the fact
that I have to undergo rehabilitation and report once a week, I have to
spend almost Four Thousand Pesos (P4,000) per month;

5. Previously, my sibling provides me financial support for my


rehabilitation, unfortunately he got sick and stopped working hence he
could no longer help me with the necessary expenses;

6. Nevertheless, I humbly seek this Honorable Court’s mercy and


consideration that I be allowed to continue my Outpatient Rehabilitation
and Treatment. Further, I undertake that I will exert more effort in
complying with my scheduled rehabilitation.

May this explanation being offered be found meritorious by this Honorable


Court, as this is advanced with all honesty and truthfulness. Hence, I am
respectfully and humbly begging the Honorable Court to allow me to continue my
Outpatient Rehabilitation and Treatment.
