ONLINE APPLICATION FOR PERMIT TO OPERATE (PTO) REQUIREMENTS

I. NEW

1. Engineer's Report prepared by the Pollution Control Officer and approved by


the President/General Manager*
a. Description of processes including the amount of raw materials and
finished products, all emissions into the atmosphere and/or material
balance.
b. Process Flow Diagram indicating all possible sources of air emission.
c. List of Air Pollution Sources Installations (APSI) and its corresponding
Air Pollution Control Facilities (APCF) with complete specifications.
d. Discussion of each APSI showing the process/mode of operations,
design criteria and efficiencies of the APCF including the types and
quantities of pollutants emitted.
2. Drawing requirements signed by the President/General Manager and duly
signed by PME/ChE or any appropriate engineer from the two*
a. Plans and Specifications of the proposed Air Pollution Source and
Control Installations (APSCI).
b. Plant and Machinery layout showing the connection of the APSI to the
APCF.
c. Vicinity Map identifying the street address, location of the plant
premises.
3. Copy of the Certificate of Accreditation of the Pollution Control Officer, or
appointment/designation as such by the Managing Head*
4. Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage
(CNC) copy*
5. Copy of DTI Registration Certificate, SEC Registration Certificate and
Cooperative Registration Certificate*
6. ECC/ CNC copy Prior to 1982/ Prior to 1996 (Gas Stations)

II. RENEWAL

1. Copy of the Certificate of Accreditation of the Pollution Control Officer, or


appointment/designation as such by the Managing Head*
2. Receive page of submitted test plan / test report with Accreditation
Certificate*
3. Copy of Recent 'Permit to Operate' issued by the Office*
4. Proof of compliance to permit conditions*
5. Copy of Self-Monitoring Reports (SMRs) for the recent year*
6. ECC/ CNC copy Prior to 1982/ Prior to 1996 (Gas Stations)
7. Hazardous Waste Registration Certificate*
8. Brochure / Picture*
