Dear Dustin Kosir,
This letter is regarding your recent COVID-19 diagnosis. Currently, the North Dakota Department of
Health, in coordination with the Centers for Disease Control and Prevention, recommends that you
isolate until it is determined that you have fulfilled the requirements for release.
Individuals who are isolating must stay home from work, school, and all social activities until deemed
non-infectious. It is also recommended that you isolate yourself in your home as much as you can to
reduce the chance of infecting others within the home.
Your period of isolation will end after a minimum of 10 days after onset of symptoms of COVID AND if
24 hours fever free (<100.4 F) AND an improvement in symptoms at that time.
Date of Test: 11/18/2020
Preliminary Release from Isolation Date: 11/28/2020 at 11:59 P.M.
It is up to you to notify your close contacts of their potential exposure to COVID-19. Please inform any
person with whom you were within 6 feet for 15 minutes or more since the 48 HOURS (2 days) before
your Test Date that it is recommended, they quarantine. You should also notify your employer, school
and/or childcare, college/university as they may have a process to identify other close contacts to you
within those environments.
The period of quarantine for close contacts is 14 days past the date of last exposure to you as the case.

testing recommendations.

The people that live with you in your house should also quarantine. See the following recommendations
