This document provides instructions for requesting information from North Carolina's Responsible Individuals List (RIL). The RIL contains the names of individuals identified as being responsible for the abuse or serious neglect of a juvenile. Child caring institutions, child placing agencies, group homes, foster care and adoption services providers can submit requests to check if prospective employees, applicants, or volunteers appear on the RIL. The requesting agency must complete all sections of the form, including information about the individual and the agency. Incomplete or illegible forms will be returned without the RIL check. Agencies can submit requests by fax or mail. The form certifies the agency's need to check the individual's fitness to care for children.

G.S. § 7B-311 authorizes the NC Department of Health and Human Print E, A, or V’s Full Name (including MI):
