AFFIDAVIT of NEAREST KIN

I, NORMA CANDELARIA S. GALVAN, of legal age, Filipino citizen and resident of


San Cornelio, Caba, La Union, after having been duly sworn to in accordance with law, do
hereby depose and say THAT:

1. I am one of the surviving and legal heirs of the late, ALBONDIO GALICA GALVAN,
who died sometime in 1958 and CRISTINA MANONGDO SOBREDO, who died
sometime on in 1969at San Cornelio, Caba, La Union, respectively;

2. I am the immediate and living nearest kin of the late, ALBONDIO GALICA GALVAN
and CRISTINA MANONGDO SOBREDO being their granddaughter;

3. I am executing this affidavit to request a copy of the Certificate of Death of ALBONDIO


GALICA GALVAN and CRISTINA MANONGDO SOBREDO in the Office of the
Local Civil Registrar of Caba, La Union and/or Office of the Philippine Statistics
Authority;

4. I am aware of the Data Privacy Act of 2012 and hereby release the Office of the Civil
Local Registrar of Caba, La Union of any violation of the said act in relation to the
release of the requested Certificate of Death/No records of the late ALBONDIO
GALICA GALVAN and CRISTINA MANONGDO SOBREDO.
