CASE:
You are having your school nursing in a rural kindergarten school. You are assigned
to a 5-year-old child, 42.5” tall and weighs 15kg. Upon observing the child, he seems to
be skinny, though well dressed, fair skin complexion, but he always runs around the class
making him sweaty. The teacher mentioned that he was always like that and has a
problem concentrating and finishing a simple task. Often times, he doesn’t like to eat
what is served by the teacher for snacks and eat his lunch if he sees them to be
vegetables even if it is mixed with meat. Sometimes he just tastes them and doesn’t finish
the food either. After class, he usually goes with her grandmother in the farm and plays
run a rounds in the rice paddies. At night he always sleeps at 9 PM and wakes up 5 AM
in the morning.
INSTRUCTIONS:
Read the cases assigned for your group. Each case contains the necessary information
you need to do the following:

a. Calculate the needed caloric intake a day. Use Tannhauser Method for DWB (5 pts)
Given:
Height: 42.5 inch Weight: 15 kg Work: School Age Child
Solution: (Tannhauser’s formula for children)
DBW= (no. of years x 2)+ 8
= (5 years old x 2) + 8
= 10 + 8
DBW= 18 kg
b. Calculate the diet prescription. (5 pts)
• Calculate the basal needs
Male: BMR = 66 + (6.23 x weight in pounds) + (12.7 x height in inches) – (6.8 x age
in years)
→ 66 + (6.23 x 33.07) + (12.7 x 42.5) – (6.8 x 5)
→ 66 + 206.03 + 539.75 – 34
→ 811.78 – 34 = 778 kcal
• Estimate physical activities= 90 %
• Calculate total energy allowance(TEA).
TEA= 778 kcal + 90% (Activity Factor)
= 868

• Total Energy Allowance(TEA) distribution


▪ Carbohydrates: 70%
▪ Proteins: 15%
▪ Fats: 15%

• For a normal diet, allot 65% of the total energy allowance for carbohydrates, 15%
proteins, and 20% fats. Thus, the corresponding energy contributors of the three
nutrients in your diet as follows

The above mention are the foods that we have prepared and chosen for the
case of a male child with 42.5” tall and weighs 15 kg. Since the child wants to play and
often has a problem concentrating and finishing a simple task these foods are essential
and helpful for the growth and physical development of the child. These foods are
crucial specially to strengthen the child’s bone when playing and to enhance his focus
when studying. And also, some foods such as milk, banana, mango, and watermelon
are used to protect the child from infections since he usually goes with his grandmother
in the farm and play in the rice paddies. And since the child is underweight, we choose
these foods to boost metabolism and increase the appetite of the child because in
every meal there is a variety of food, at the same time snacks to ensure that the child
will be tempted to go to the table and eat the prepared foods. Thus, his weight may
increase and he can have good nutrition. Besides, these foods increase the clarity of
thought and ability to recall information which will help and boost the child to do his
given task. Also, the cucumber and watermelon will replenish the fluid that was taken
out in the body of the child due to sweating to avoid dehydration. The foods that were
prepared also contains low calories and carbs to promote digestive health and
maintain a healthy weight for the child.
