American Society of
Anesthesiologists (ASA)
Score
American Society of Anesthesiologists (ASA) Score is a
global score that assesses the physical status of patients
before surgery.
It is sometimed refereed to as ASA-PS, because it is a
measure of ‘physical status’.
ASA 1 A normal healthy patient.
ASA 2 A patient with mild systemic disease.
ASA 3 A patient with severe systemic disease.
ASA 4 A patient with severe systemic disease that is a
constant threat to life.
ASA 5 A moribund patient who is not expected to
survive
There are modifications – the addition of “E” for an
emergency, the addition of “P” for pregnancy, and ASA 6
for organ retrieval in brain-dead patients.
The ASA Score is a useful global measure of health.
Table 1. ASA Scores.
Class Physical status Example
I A completely healthy patient A fit patient with an inguinal hernia
II A patient with mild systemic Essential hypertension, mild diabetes
disease without end
organ damage
III A patient with severe systemic Angina, moderate to severe COPD
disease
that is not incapacitating
IV A patient with incapacitating Advanced COPD, cardiac failure
disease that
is a constant threat to life
V A moribund patient who is not Ruptured aortic aneurysm, massive
expected pulmonary
to live 24 hours with or without embolism
surgery
E Emergency case
Table 2. CEPOD Scores
Grade 1 Elective
Operation at time to suit both surgeon and patient
Grade 2 Scheduled
Operation within 24 hours. Delayed operation after resuscitation
Grade 3 Urgent
Operation between 1 and 3 weeks. Early surgery preferred, but not
life saving
Grade 4 Emergency
Operation within 1 hour. Immediate operation or resuscitation
simultaneous with surgical treatment
