I, Raaaa Jr., of legal age, single, Filipino, and resident of Don Jose,Santa Rosa, Laguna, do
hereby appoint, MSSSS dY of legal age, single, Filipino, and residing at B 100 L 101 Ph 20 Co
Residences, abc, mANILA as my true and legal representative to act and in my behalf and stead and to
perform the following acts:

To transact, submit, claim, sign, and negotiate with the Bureau of Internal Revenue for
the Case Monitoring System and for the closure of the business under the name of

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority to
do and perform all and every act and thing whatsoever, requisite or necessary to be done in and about
the premises and hereby ratifying and confirming all that my said attorney-in-fact shall lawfully do or
cause to be done by virtue of these presents.
