AFFIDAVIT
I, GERLITO S. ROGIO, of legal age, Filipino, married, and a resident
of Barangay Pook-Interior, Kalibo, Aklan, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That I am the brother of landowner, Hildegario S. Rogio; and at the


same time the actual tiller of parcels of land designated as Lot 2361-
A and Lot 3177-C-1, both located at Pook, Kalibo, Aklan. The same
are covered by TCT No. T-22786 and TCT No. T-27965,
respectively;

2. That I have actual knowledge of the description of the parcels of


land since I am the actual tiller thereof;

3. That I am requesting for the condonation and writing off of unpaid


irrigation service fee and also for the certificate of exemption in the
payment of irrigation service fee with my brother, Hildegario S.
Rogio;

4. That the Lot Number No. 2631 recorded in the NIA is one and
the same as Lot No. 3171C-1; and Lot No. 2622 B in the NIA is
one and the same as Lot No. 2631-A;

5. That I am executing this affidavit to identify the foregoing parcels of


land as one and the same lots recorded in the NIA and to attest to the
truthfulness of the foregoing;
