AFFIDAVIT

I, ROSITA L. PEDRAZA, 57 years old, married, Filipino, and a resident


of Del Carmen Village, Matina Aplaya, Davao City after being duly sworn in
accordance with law, hereby depose and say THAT :

1. I am the mother of LILIBETH P. FUKUMOTO who is married to


TOSHINORI FUKUMOTO and who are presently in Japan;

2. They have a child named LITO FUKUMOTO who is twelve (12) years
old;

3. LITO FUKUMOTO was under my care and custody since he was one
(1) year old;

4. I will accompany my said grandchild in going to Japan to visit his


parents;

5. I am executing this affidavit in order to attest to the truth of the


foregoing facts and for whatever purpose it may serve.
